Your contact information is stored in a secure database protected by firewalls and anti-intrusion software. Clinical information is transmitted and stored through HIPAA-compliant platforms (Practice Better for portal communications; HIPAA-compliant file-sharing services for imaging uploads). We implement administrative, physical, and technical safeguards appropriate to protect personal and health information. While we implement reasonable safeguards, no system can guarantee absolute security.
Where We Send Your Data
Visitor comments may be checked through an automated spam detection service. Clinical data is not shared with third parties except as required to provide services, coordinate care at your request, or comply with legal obligations. Imaging files submitted for NeuroSignal Review are processed on secure, HIPAA-compliant systems.
Children’s Privacy
Our website and services are not intended for or directed to individuals under the age of 18. We do not knowingly collect personal information from children under 18 in compliance with the Children’s Online Privacy Protection Act (COPPA). Our services are available to adults aged 18 and older only.
Image Optimization (Smush Plugin)
Our website uses the Smush plugin to optimize images for web display. Smush sends images to WPMU DEV servers for optimization, including EXIF data transfer. EXIF data is either stripped or returned as-is and is not stored on WPMU DEV servers. Smush uses the Stackpath Content Delivery Network (CDN), which may store web log information (IP addresses, user agent, referrer, location, and ISP information) for up to 7 days. Files served by the CDN may be stored and served from countries other than your own.
Changes to This Privacy Policy
The Practice reserves the right to update this Privacy Policy at any time. Continued use of the website after changes are posted constitutes acceptance of the revised policy. Material changes will be noted by updating the “Last Updated” date above.

The Practice is a physician-led telehealth practice operated by Atif Hashmi, MD, a board-certified neurologist. The Practice operates under NeuroSignal Insights LLC and offers two services:
- The Cognitive Resilience Program: A 12-month physician-led telehealth program providing limited-scope physician consultations focused on brain-health counseling, risk-factor review, and evidence-based lifestyle recommendations.
- NeuroSignal Review: A limited-scope physician consultation for expert neuroimaging review of brain and spine MRI and CT studies, including a written advisory report and live video consultation.
Both services establish a limited-scope physician-patient relationship for specified services only.

3. Eligibility
The Cognitive Resilience Program is available only to adults aged 18 and older who can consent for themselves.
For the NeuroSignal Review, a legally authorized representative may act on behalf of a patient, subject to verification of legal authority.
We do not provide services to minors. Patients must be physically located in Ohio (or a state where Dr. Hashmi is licensed) at the time of every telehealth session.
4. Scope of Services
Both the Cognitive Resilience Program and NeuroSignal Review establish a limited-scope physician-patient relationship for specified services only. This means:
- The physician-patient relationship is limited to the services described in your enrollment or booking documents.
- The Practice does not provide primary care, emergency care, disease management, or medication prescribing.
- Findings suggestive of a clinical condition requiring diagnosis or treatment will be referred to your PCP or an appropriate specialist, not managed within our programs.
- The Practice reserves the right to decline or terminate services if the patient’s needs exceed the scope of the services offered.
- Coordination with your treating physician is encouraged and facilitated at your request.
5. Telehealth Services
All services are delivered via telehealth (live video consultation). The Practice does not operate an in-person office. Audio-only consultations are not available. Key requirements:
- Identity verification occurs at the start of every telehealth session.
- The patient must confirm they are physically located in Ohio (or a licensed state) at the time of each session. If the patient is discovered to be outside an authorized state, the session will be terminated and handled in accordance with the applicable service agreement and financial policy.
- The patient must provide their nearest emergency facility and an emergency contact prior to the consultation.
- Neither party may record the consultation without mutual written consent.
6. Use of Content
The Practice authorizes you to view and use a single copy of the Content on this website solely for your personal, non-commercial use. Content on this website is protected by copyright under United States and international law. Title to the Content remains with the Practice or its licensors. Any unauthorized use of the Content is a breach of these Terms and may violate copyright, trademark, and other laws. All rights not expressly granted herein are reserved.
7. User Submissions

Clinical information submitted through our patient portal, intake forms, and imaging upload systems is governed by HIPAA, our Notice of Privacy Practices, and the terms of your consent documents.
8. Patient-Submitted Material
For NeuroSignal Review, the patient is responsible for:
- Ensuring they have lawful access to submitted imaging studies.
- Providing documentation of legal authority if submitting images on behalf of a dependent or represented adult.
For the Cognitive Resilience Program, all information submitted through intake forms, questionnaires, and portal messages becomes part of the patient’s medical record as described in the consent documents.
9. Medical Records
Because this is a physician-led practice, Dr. Hashmi creates and maintains medical records for all clinical services. Medical records include intake forms, session notes, test results and interpretations, advisory reports, completion reports, and clinical portal communications. Records are retained for a minimum of seven (7) years in compliance with Ohio law. Patients may request copies of or amendments to their medical records through Practice Better or by contacting the Practice directly. Telehealth sessions are not recorded and no audio or video recordings are retained.
10. Limitation of Liability
The use of this website and the Content is at your own risk. The website and Content are provided on an “as is” basis. To the fullest extent permitted by law, the Practice disclaims all warranties, whether express or implied, including but not limited to warranties of merchantability, non-infringement of third-party rights, and fitness for a particular purpose.
In no event shall the Practice be liable for any damages (including incidental, consequential, or punitive damages, personal injury, lost profits, or damages resulting from lost data or business interruption) resulting from the use of or inability to use this website or Content, whether based on warranty, contract, tort, or any other legal theory. The Cognitive Resilience Program requires a three-month minimum commitment, cancellable anytime thereafter with written notice. NeuroSignal Review offers a full refund if imaging cannot be reviewed for any reason (insufficient quality, wrong study type, or ineligibility). Specific cancellation and refund terms are detailed in each service’s enrollment or booking documents.
13. Indemnification
You agree to defend, indemnify, and hold harmless NeuroSignal Insights LLC, its officers, employees, and agents from any claims, liabilities, and expenses (including reasonable attorneys’ fees) arising from your violation of these Terms of Use or your use of this website. This provision applies to website use only and does not modify or limit any rights or obligations arising under applicable medical malpractice law or in relation to clinical services provided by the Practice.
14. Jurisdiction and Governing Law
These Terms of Use are governed by the laws of the State of Ohio, without regard to conflict of law principles.
